At Sanctuary House, we believe everyone deserves the opportunity to live with dignity, safety, and independence in the community of their choice. Our Transition Management Services (TMS) help make that possible for individuals participating in North Carolina’s Transition to Community Living (TCL).

TMS is a specialized rehabilitation service that assists individuals transitioning from restrictive or institutional settings into independent housing, and supports them in maintaining housing successfully over time. Through a highly personalized and practical approach, we help individuals build the skills, knowledge, and community connections needed to thrive.

The TMS Process

We begin by working closely with each individual to:

  • Identify housing preferences and any barriers to a successful transition or long-term tenancy.
  • Assess independent living and social skills needed to maintain housing, using a functional assessment tool.
  • Identify resources to cover essential one-time expenses such as security deposits, moving costs, furnishings, adaptive aids, and home modifications.

Our team provides hands-on assistance to help individuals successfully move into and sustain housing:

  • Support with housing applications and the housing search process, including identifying rental units and landlord partners, completing applications, and arranging transportation during the search.
  • Ensure the home is safe and ready for move-in, including support with setting up utilities and household items.
  • Provide early identification and intervention for behaviors that could jeopardize housing, such as late rent or lease violations.
  • Educate tenants on their rights and responsibilities, and foster relationships with landlords to promote long-term housing stability.
  • Assist in resolving landlord or neighbor disputes and reconnect individuals to community resources if housing is at risk.
  • Assist with housing recertification, compliance with lease agreements, and general household management.
  • Act as a primary contact for landlords regarding tenancy issues.

We help individuals access financial resources and build money management skills:

  • Assist in obtaining identification documents, repairing credit, and correcting criminal records when applicable.
  • Support applications for SSI/SSDI, Medicaid, Special Assistance In-Home, food stamps, veterans benefits, and other entitlements.
  • Provide coaching on budgeting and bill payment, and monitor financial needs monthly or more frequently if needed.
  • Help manage utility expenses and address issues related to utility arrears.
  • Manage Special Assistance In-Home benefits in coordination with local social services departments.

We assist individuals in developing or improving essential life skills for independent living:

  • Self-care and personal safety.
  • Meal planning, grocery shopping, safe food preparation and storage.
  • Clothing care and household cleaning.
  • Safe and effective use of various transportation modes.

Our team supports overall well-being by helping individuals:

  • Manage medications and pharmacy services.
  • Navigate personal health needs and connect to healthcare services.
  • Maintain healthy nutrition and physical activity routines.
  • Access self-help groups and online resources for managing symptoms.
  • Use technology to stay connected with service providers and natural supports.
  • Develop personal crisis management plans (WRAP, WHAM, PAD), relapse prevention plans, and coping strategies for managing symptoms.

We encourage full participation in community life by helping individuals:

  • Build social skills and friendships.
  • Identify personal interests and develop plans for engaging in meaningful activities.
  • Develop confidence in social situations, such as hosting get-togethers or going out to restaurants, movies, and events.
  • Learn to use community resources (phone, internet, newspapers) to stay informed about local opportunities.

TMS is available to adults who are:

  • Participating in the Transition to Community Living (TCL), and
  • Approved for a state-funded housing slot.

Participation in TMS is coordinated through your Person-Centered Plan (PCP) and may be provided alongside other mental health services.
